What to Bring
To help make your first visit with us effective and productive,
there are several items that you should bring with you. This helps
us make sure that your first visit with us is thorough and complete.
It's important to bring any and all insurance cards, including
your major health plan, any supplements, etc. Also, if your insurance
plan requires a referral from your primary care/family doctor,
remember that it is your responsibility to make sure the referral
is arranged prior to your appointment. If your referral is not
arranged, we may need to reschedule your appointment.
If your visit to us is the result of a worker's compensation
case or a motor vehicular accident, we must have a letter from
the insurance carrier that includes the claim number, contact
person and telephone number, address where to submit claims and/or
medical information, and any applicable policy numbers. We will
need this information so that we can begin your physician care.
X-rays, MRIs, Bone Scans, Myelograms and Tomograms are all an
essential part of spine care. If you have had any of these studies
performed, it is imperative to your evaluation and treatment that
our physicians have the films and the reports the day of your
appointment. We will need to know all medications you are currently
taking, including how many and how often. Also, please let us
know about any medication allergies you may have.
It is Spine Institute’s policy that all patients under the age
of 18 must be accompanied by a parent or guardian.

To best serve you, our office visits are scheduled by appointment.
We offer office hours Monday through Friday. You can make an appointment
by calling (502) 584-7525 between 8:15 a.m. and 4:45 p.m.
We realize your time is valuable, and we will make every effort
to remain on schedule. Occasionally, our physicians may be disrupted
by medical emergencies. When this occurs, we hope you will understand
and remain patient. Of course, we will make every effort to inform
you of unusual delays.
Arriving on time for your appointment will help with any unnecessary
delays. If you find that you will be unavoidably late, please
contact the office. Should you be unable to keep your appointment,
please notify our staff at least twenty-four hours in advance.
Telephone Calls and Prescription
Refills
Sometimes, you may need to call our office to clarify or obtain
further medical information. If your call is urgent, please tell
the telephone operator at once, and he or she will make sure that
your situation is addressed immediately.
If your call is of a non-urgent nature, and a clinical assistant
is available, we will make every effort to address your call immediately.
However, if a clinical assistant is with another patient, your
call will be sent to an automated message system. Just leave your
name, the patient's name (if different), the physician who is
treating the patient, a telephone number where you can be reached,
and a brief message. One of our clinical assistants will return
your call as soon as possible.
Medications can often be refilled via the telephone. When your
medication is running low, the most efficient way to obtain a
refill is to have your pharmacy contact our office so the pharmacist
can receive authorization from us. Remember, you should allow
twenty-four hours for your request to be processed. Also, for
your safety, we usually will not accept refill requests during
evenings and weekends because your medical records are not available.
Insurance and Billing
We are glad to file your insurance claims for you. Our billing
office staff is available Monday through Friday to help answer
your billing questions, and can be reached at (502) 584-8002.
In an effort to help control costs, we ask that all co-payments
be made at the time of service. For your convenience, we accept
cash, personal checks, MasterCard and Visa. If your visit is not
covered under any insurance plan, please call our office prior
to your visit to discuss payment options.
All patients with outstanding balances will receive a monthly
statement. Delinquent accounts may be sent to a collection agency
if no attempt has been made to settle the account.
Parking
The first five floors of our building are designated for parking.
For your convenience, valet parking is available in front of the
building, and parking for persons with special needs is also available.
After your visit, our staff will be happy to validate your parking
ticket. |
